Membership Create

Join an active tenant. An active membership returns 409. A revoked membership returns to active using the same membership_id, clears revoked_at and advances its continuous version. Globally disabled or suspended people cannot join. Staff can join only the platform tenant; any other tenant returns IDENTITY_STATE_INVALID.
